NDIS Support After Discharge from Nepean Hospital
Leaving hospital with a disability is where NDIS supports either start smoothly or fall apart. We set up plan management, support coordination and in-home supports for participants discharging from Nepean Hospital.

Tell us what you need and we will call you back. No obligation.
Nepean Hospital at Kingswood is the major hospital for Penrith, the Blue Mountains, Hawkesbury and Lithgow, and its rehabilitation clinics – stroke, Parkinson’s disease, multiple sclerosis, motor neurone disease, spasticity and amputee care – are often where NDIS planning starts. Because the most severe trauma and inpatient brain injury rehabilitation are networked to Westmead, some participants come home to the Penrith area after time in more than one hospital.
As a registered NDIS provider, we help families in Penrith, St Marys, Mount Druitt and Blacktown line up plan management, support coordination and in-home supports so the move home – often a long way from where treatment happened – does not stall.
About Nepean Hospital
Nepean Hospital is located at Derby Street, Kingswood and is part of the Nepean Blue Mountains Local Health District.
Nepean Hospital is the major hospital for Penrith, the Blue Mountains, Hawkesbury and Lithgow, and a teaching hospital of the University of Sydney. It is classified as a regional trauma service rather than a major trauma service, which means the most severe multi-trauma cases are networked to Westmead.
Services That Matter Most for NDIS Participants
- Rehabilitation medicine: Outpatient specialist clinics covering stroke, Parkinson’s disease, multiple sclerosis, cerebral palsy, motor neurone disease and post-spinal-surgery rehabilitation.
- Amputee and orthotics clinic: A regular specialist clinic, directly relevant to participants requiring prosthetics and assistive technology.
- Spasticity clinic: A dedicated clinic that often connects to NDIS-funded therapy and equipment needs.
- Neurosurgery, aged care and mental health: Including the Nepean Mental Health Centre and a child and adolescent mental health unit.
- A Changing Places facility: A fully accessible bathroom for people with high support needs, delivered as part of the current redevelopment.
Worth knowing: Nepean does not have a brain injury rehabilitation unit. Participants from this catchment needing inpatient brain injury rehabilitation are generally referred to Westmead.
Getting NDIS Supports in Place Before Discharge
The NDIA runs a formal hospital discharge pathway, and the earlier it starts the more likely a discharge date holds. An NDIA hospital liaison officer works with the treating team on any plan changes, a support coordinator lines up providers, equipment and home modification quotes, and a plan manager makes sure new providers can be paid from day one.
We explain the whole process – including why discharges get delayed and what we do in the first few weeks at home – in our guide to NDIS support after hospital discharge.
Getting Around Nepean Hospital
Kingswood station is roughly 1.2 kilometres from the hospital, about a fifteen minute walk, and bus routes 774, 775 and 776 drop off directly outside on Derby Street. There are ten-minute drop-off and pick-up zones on campus that taxis, rideshare and community transport can use. The redevelopment continues, so signage and entry points change periodically.

Frequently Asked Questions
Can NDIS planning start from Nepean Hospital’s rehabilitation clinics?
Yes. Nepean’s outpatient rehabilitation clinics for stroke, Parkinson’s disease, multiple sclerosis, cerebral palsy, motor neurone disease and post-spinal-surgery rehabilitation are often where NDIS needs first become clear. Plan management and support coordination can be set up so therapy recommendations turn into funded supports.
What happens if the person is transferred from Nepean to Westmead?
Nepean is a regional trauma service, and inpatient brain injury rehabilitation for this catchment is generally provided at Westmead. Supports can be arranged while the person is at Westmead so they are ready when they come home to the Penrith area.
Can you help with prosthetics and equipment after Nepean’s amputee and orthotics clinic?
We handle the NDIS side: support coordination to find suppliers and progress quotes, and plan management so suppliers are paid on time. The clinical prescription stays with the clinic team.
How do we get to Nepean Hospital?
Kingswood station is about 1.2 kilometres away, buses 774, 775 and 776 stop outside on Derby Street, and there are ten-minute drop-off zones on campus that community transport, taxis and rideshare can use.
