NDIS Support After Discharge from Concord Hospital
Leaving hospital with a disability is where NDIS supports either start smoothly or fall apart. We set up plan management, support coordination and in-home supports for participants discharging from Concord Repatriation General Hospital.

Concord Repatriation General Hospital has the strongest rehabilitation and aged care profile in the Sydney Local Health District, hosts one of only two adult severe burn units in NSW, and is home to the National Centre for Veterans’ Healthcare. It is also a common stop for subacute rehabilitation, so many participants arrive at Concord from another hospital and go home from here.
As a registered NDIS provider, we help families across Burwood, Canada Bay, Strathfield, the Inner West and Canterbury set up plan management, support coordination and in-home supports while rehabilitation is still under way, so the last discharge is the smooth one.
About Concord Repatriation General Hospital
Concord Repatriation General Hospital is located at Hospital Road, Concord and is part of the Sydney Local Health District.
Concord is a principal referral and major teaching hospital with the strongest rehabilitation and aged care profile in its district. Following the completion of its eight-storey clinical services building it has 563 beds, and it hosts services that exist in only one or two places in the state.
Services That Matter Most for NDIS Participants
- NSW Statewide Severe Burn Injury Service: One of only two adult severe burn units in New South Wales.
- Aged care and rehabilitation: A defining strength of the hospital, including geriatric medicine outpatients and a psychogeriatric clinic.
- Specialised rehabilitation gyms: Expanded rehabilitation therapy areas delivered in the recent redevelopment.
- National Centre for Veterans’ Healthcare: Australia’s first, an outpatient service for veterans and their families.
- Mental health services: Including the Concord Centre for Mental Health.
Worth knowing: Concord does not have a brain injury rehabilitation unit or a spinal cord injury unit, but it is a common destination for subacute rehabilitation before a participant returns home.
Getting NDIS Supports in Place Before Discharge
The NDIA runs a formal hospital discharge pathway, and the earlier it starts the more likely a discharge date holds. An NDIA hospital liaison officer works with the treating team on any plan changes, a support coordinator lines up providers, equipment and home modification quotes, and a plan manager makes sure new providers can be paid from day one.
We explain the whole process – including why discharges get delayed and what we do in the first few weeks at home – in our guide to NDIS support after hospital discharge.
Getting Around Concord Hospital
Rail access is limited. Buses 410, 458 and 460 run on Hospital Road. Rhodes station is a fifteen minute walk or reachable by bus, and Concord West is about twenty minutes on foot. Strathfield and Burwood are both around four kilometres away with bus connections. All of those stations are wheelchair accessible, but the last leg to the hospital almost always involves a bus.

Frequently Asked Questions
Why do so many people go home from Concord after another hospital?
Concord is a common destination for subacute rehabilitation, so many participants transfer from other hospitals in the district before going home. It is a good point to finalise NDIS supports because the person’s needs at home are clearer by then.
Can you help after Concord’s Statewide Severe Burn Injury Service?
Yes. Concord hosts one of only two adult severe burn units in NSW, and recovery often continues with therapy and equipment at home. Support coordination and plan management help get those supports funded and delivered.
Do you support veterans who are also NDIS participants?
Yes. The National Centre for Veterans’ Healthcare at Concord is an outpatient service for veterans and their families, and NDIS supports can be arranged alongside it for veterans who are NDIS participants.

Buses 410, 458 and 460 run on Hospital Road. Rhodes station is about fifteen minutes on foot or reachable by bus, and Concord West is about twenty minutes on foot. Strathfield and Burwood are around four kilometres away with bus connections.
